If you have hair loss from any medical condition such as Chemotherapy, Alopecia, Trichotillomania, psoriasis or other medical conditions that has caused you to losse your hair, you may be eligible to receive a full or partial refund or financial assistance for purchasing a "Full Cranial Prosthesis." You might also be eligible to receive a voucher or grant from one of the organizations listed below.

Request Your Health Insurance Provider to Cover the Cost of Your Lace Wig - Cranial Prosthesis
BEFORE you purchase the lace wig, you MUST contact your health insurance provider to see if a “cranial prosthesis” is covered and what the procedure is to obtain full or partial refund for purchasing a “cranial prosthesis.” MAKE SURE when you request a
prescription from your physician, that he refers to the full lace wig as a "cranial prosthesis.”
Note: You MUST use this terminology "cranial prosthesis” because this is the terminology used for a full lace wig and it might enable the wig to be covered under your medical insurance policy.
Health insurance providers may cover between 80-100% of the cost for your “cranial prosthesis.”
DO NOT request coverage for a wig because more than likely, your claim will be denied. You MUST request a "cranial prosthesis."
Call Your Health Insurance Provider to ask if they cover “cranial prosthesis” for medical hair loss. If it is covered, ask them to explain their procedure for obtaining payment or reimbursement for “cranial prosthesis.”
D' Luxe Lace Wigs DOES NOT process insurance claims and we DO NOT accept prescriptions. You need to find out the exact requirements from your health insurance provider regarding their procedure for payment or reimbursement. If you are using health
insurance for payment, please let us know in advance so that we can make sure your invoice reads correctly or otherwise, your health insurance provider might not pay.
Note: You MUST follow all instructions given to you by your health insurance provider or you may not qualify for the reimbursement. Make sure you keep copies of all of your documentation and invoices for tax exempt purposes.
Grants or Vouchers
Contact one of the following organizations to apply for a Grant or Voucher BEFORE you purchase the lace wig. The grant will not be awarded or even considered if you already purchased the lace wig.
Please contact one of the following organizations to determine if they will assist you in purchasing your full or front lace wig (cranial prosthesis) with a Grant or Voucher:
1. National Alopecia Areata Foundation (NAAF)
NAAF established the Ascot Fund, which is a special program that provides financial assistance toward the purchase of a full or front lace wig to individuals who have alopecia and are struggling financially.

2. American Cancer Society
If your health insurance provider does not cover the cost of your lace wig or only covers a portion of the cost, you may qualify for a voucher through the American Cancer Society.
In order to qualify for this voucher, you must meet certain financial guidelines. You MUST also apply for this voucher BEFORE you purchase the lace wig. The voucher will not be awarded or even considered if you already purchased the lace wig.
If the American Cancer Society accepts your claim, they will issue you a voucher in the amount determined by the American Cancer Society to help defray the cost of your lace wig.
